Check/Reimbursement
Request:
Use this form when you have purchased a product or service for the
church for which you would like to obtain reimbursement. Please be
sure you have had prior approval from the appropriate board member,
committee chair or staff member before you submit the form.
Contribution Form:
Use this form when you have purchased or donated a product or service for
the church for which you do NOT want reimbursement, but DO want
acknowledgement and/or a receipt for tax purposes. We strongly
encourage anyone who has donated something to the church to fill in this
form. Although you may not care whether you are acknowledged for your
donation, it helps us when it comes time to make out our annual budget.
Scholarship Form:
Every year the UUCA sets aside money to provide scholarships for children
and adults who might not otherwise be able to attend activities sponsored by
the UUA or the district. Use this form as part of the process of
applying for one of these scholarships.
